Please read the following carefully.If you answer YES to any statement that indicates a medical condition, or NO to any statement that indicates eligibility, you do not qualify to use our Hyperbaric Oxygen Chamber at this facility.In these cases, we recommend consulting with your physician and using a hospital-grade Hyperbaric Oxygen Chamber under a physician's prescription.
ABSOLUTE CONTRAINDICATIONS: Please review the following conditions. Select any that apply to you:
- I HAVE NOT had a heart attack in the last 6 months.
- I DO NOT have a pacemaker.
- I HAVE NOT had a heart bypass or valvular disease within the past 6 months.
- I DO NOT have congestive heart failure.
- I DO NOT have chronic obstructive pulmonsty disease (COPD).
- I DO NOT have an intrathecal pain pump or any electro-stimulation implant device (i.e., spinal stimulator implant).
- I DO NOT have any chronic or acute kidney conditions.
- I AM NOT pregnant.
- I HAVE NOT had radiation in the last year.
RELATIVE CONTRAINDICATIONS: *
- I DO NOT have a history of seizure disorders.
- I DO NOT have any inner ear or sinus disorders.
- I DO NOT have any central circulatory disorders (such as deep vein thrombosis).
- I DO NOT have any open wounds, sores or healing disorders.
